ChroniCare is a chronic disease management software business who uses medical diagnostic software to help patients. A large and growing portion of the world population suffers from chronic diseases. Despite global healthcare delivery being at the point of rupture, technology is only sparingly used in healthcare delivery. Our software helps patients understand their diseases, how to treat them, and how to manage them. Our diagnostics are designed by leading doctors and completely customized to the individual patient. Failure to treat and manage chronic diseases results in bad healthcare outcomes. This directly and often tragically impacts the patients and their loved ones. In addition, it has knock-on effects for insurers, corporations and health care providers. We work with all stakeholders in the health landscape including patients, families, insurers, governments, doctors, healthcare professionals and pharmacists. ChroniCare are launching AFCare.net to help patients with Atrial Fibrillation in the third quarter 2017. We are working with the AF Association, the world's preeminent Atrial Fibrillation charity. We were founded by medical (and technology) professionals, and all diagnostics are in accordance with National Institute for Health and Clinical Excellence (NICE) guidelines.
